Patient Choices Vermont (PCV), founded in 2002, is a non-profit organization dedicated to end-of-life choice. PCV worked for eleven years to achieve enactment of the law entitled Patient Choice at End of Life (Act 39). After the law passed, PCV became the steward of medial aid in dying in Vermont, as the most trusted source for accurate information and education. PCV is a 501(c)(3) organization.
Mission: PCV works to enable people who might otherwise be entering a phase of fear, pain and trauma for themselves and their loved ones the freedom to step off that path and make conscious choices about where, when and with whom they will live their last days and make their final farewell. This mission is about autonomy, dignity, equal access and individual values. Our vision is a world where all relevant information is available and everyone is able to choose the end-of-life care that reflects their values and priorities. We help open the opportunity for powerful personal choice.
Important liberties such as the choices Act 39 provides require continued vigilance. PCV educates new legislators and sustains its legislative watch function in order to ensure that Act 39 is in place for the long term.
501(c)(3): Patient Choices Vermont is a 501(c)(3) organization, meaning that contributions qualify as tax deductible. The formal name of the organization is Vermont End of Life Choices, Inc. Patient Choices Vermont is the organization's doing business as (DBA) name.

Finance: PCV's 2024 budget is $125,000, which funds part-time staff positions of President and Program Manager, along with funds for the PCV website, educational programs, development of the networks of medical providers who serve end-of-life patients, and PCV's helpline services. PCV is at an inflection point where we can only pursue our pro-active mission and meet the growing needs if we transition to a more robust structure. It is time for PCV to transition from its founder/volunteer structure to a full-time staff with the staying power to ensure that medical aid in dying is available in Vermont for the long-term. This plan requires growth of the annual budget from $125,000 to $375,000. We have laid a strong foundation. Our team includes doctors, social workers, lawyers, nurses and hospice professionals. All of the current team will remain involved with the organization.

Honor Donations:
You can easily designate Patient Choices Vermont by listing our organization in your obituary, along with a link to this Donate Page and our PO Box address for checks. Each time we designate PCV for such contributions, we will be informing others about medical aid in dying and helping ensure that end-of-life choice is available for the long-term.
Donate Stock or Other Financial Securities:
By giving appreciated securities you can avoid paying capital gains taxes, receive a tax deduction on the full value of the gift, and support Patient Choices Vermont at a lower cost to you than if you made a cash gift. If you would like to make a stock donation, please contact us for transfer instructions.
Give From Your IRA:
People age 70½ and older can donate up to $100,000 directly to Patient Choices Vermont from their individual retirement accounts (IRAs) without it being counted as taxable income. The gift can be used as your required minimum distribution. Your financial institution needs to issue the check directly to PCV, and the gift must be made by December 31 of the tax year. Contact your provider or visit their website to obtain a simple form to fill out.
Leave a Legacy:
You can leave a lasting legacy in support of end-of-life choice by naming Patient Choices Vermont as a beneficiary in your will or living trust, life insurance or retirement account. You don't have to be wealthy to do this. A donation of any amount can be designated.
